The scales in the Clinic’s Z Ellis office are Silent Hill Townfall’s first puzzle that asks you to leave the room, follow several clue trails and return with a complete set of objects. The basic answer is straightforward once every item is in hand: put the broken stethoscope on the scale for not listening, the incorrect prescription on the scale for making mistakes, and Zoe’s ID card on the scale for no longer being fit for the role.

The tricky part is that the Clinic note presents the solution as a series of professional failings rather than item names. You begin with only one matching object, while the other two sit behind routes and smaller puzzles elsewhere in town. This walkthrough explains where the scales are, how to add both needed destinations to the map, what each detour requires and why the final placements work.

Scales puzzle answer at a glance

  1. “I didn’t listen closely enough” — place the broken stethoscope.
  2. “I made mistakes and couldn’t focus on the task at hand” — place the incorrect prescription.
  3. “I’m not fit to work in this role any longer” — place Zoe’s ID card.

Once all three scales have the correct item, a short cutscene plays and the game moves forward. If you only need the answer, that is the full combination. If an object is missing, the sections below cover the required routes.

Where to find the Clinic scales

The puzzle is in the Z Ellis office in the Clinic, the same room connected to the Clinic reception door-code trail. As you enter, three scales sit on boxes directly ahead of you. A note beside them contains several underlined statements. Those lines are not merely flavour text: each is a description of one object you must recover and place.

The relevant statements concern failing to listen closely, making mistakes and being unfit to continue in a job role. The note ends by asking you to demonstrate understanding. In practical puzzle terms, that instruction means matching a physical object to each statement rather than trying to calculate a weight or balance the scales numerically.

That distinction matters. A scale puzzle might normally imply comparing mass, but Townfall uses the three scales as separate slots. The challenge is interpretive: identify the medical or workplace item that best represents each lapse, then put it in its corresponding position.

What you already have: Zoe’s ID card

By the time you reach this puzzle, you should already have Zoe’s ID card, obtained by completing the organ box puzzle. It corresponds to the statement:

“I’m not fit to work in this role any longer.”

An ID card represents the holder’s professional identity and authorization. In the context of the note’s admission of being unable to carry on in the role, it is the intended match for the third scale. You can leave it until you have gathered the other items, or place it immediately if the game allows you to interact with that scale; either way, it belongs in the final position.

Two objects remain: the item tied to listening and the one tied to mistakes. The Z Ellis office contains the leads that put both locations on your map.

How to unlock Jess’s Flat and the Pharmacy on the map

Before setting out, inspect the items in and around the Clinic area carefully. The two destinations are revealed through different interactions:

  • Jess’s Flat: use the computer in the Z Ellis office and read the email titled “Where are you”. This adds Jess’s Flat to the map.
  • Pharmacy: first obtain and use the Clinic reception door code from the computer trail. Then listen to the reception answerphone to have the Pharmacy added to the map.

The destinations can be handled in either order. Neither route changes the final scale arrangement, and both ultimately lead back to the Clinic with one key object. If you want a simple route plan, tackle the Pharmacy when ready to solve its locker combination, then visit Jess’s Flat for the stethoscope. But there is no evidence that the game requires that sequence.

Pharmacy route: get the incorrect prescription

The Pharmacy provides the object for the clue about mistakes. You need the scalpel to gain entry, so make sure it is in your inventory before making the trip. Inside, the important obstacle is a locker secured by a combination.

This locker puzzle uses two layers of information. Numbers shown on posters provide the digits, while the TV signal tells you the order in which to arrange them. In other words, do not assume the posters’ physical left-to-right order is the code. The television signal functions as the sequencing clue that turns the collected numbers into the correct combination.

Opening the locker gives you an incorrect prescription. That is the required scale item for:

“I made mistakes and couldn’t focus on the task at hand.”

The match is deliberately literal. A prescription is a medical instruction, and an incorrect one is a tangible consequence of a mistake in that work. It belongs on the middle scale, not the one associated with listening. Keep the prescription until returning to the Z Ellis office.

A note on the Pharmacy locker puzzle

Townfall’s clue structure here is worth reading carefully because it separates what the digits are from how they are ordered. When a puzzle offers numbers in one place and a broadcast, image or signal somewhere else, the second clue often supplies arrangement rather than additional digits. If your combination is being rejected, revisit both parts of that relationship: the poster numbers and the TV-guided sequence.

Jess’s Flat route: get the broken stethoscope

Jess’s Flat contains the item tied to the note’s line about failing to listen. As with the Pharmacy route, the scalpel is needed: use it to open the gate to St Amelia’s Park.

On the way to Jess’s Flat, stop at No. 16. The route through this building is important because it contains the key needed to reach the actual destination. Go to the upstairs flat and look for Jess’s backdoor key on a table near the window. With that key collected, approach Jess’s Flat from the rear and enter through the back door.

Inside Jess’s Flat, find the broken stethoscope on the bed. This is the object needed for:

“I didn’t listen closely enough.”

The connection is again direct but thematic: a stethoscope is a device for listening to bodily sounds. Its damaged condition reinforces the note’s confession that the person failed to listen properly. Carry it back to the Clinic for the first scale.

Optional detour: the No. 16 lock box

While taking the Jess’s Flat route, there is a useful extra interaction to avoid overlooking. The ground-floor flat at No. 16 has a lock box, and a key for that box is located in a kitchen drawer at Jess’s Flat. Pick up the key while you are inside Jess’s place, then open the lock box as you travel back.

This lock-box task is separate from the three objects required for the scales. It does not alter the solution, but collecting its key while you are already at Jess’s Flat prevents an unnecessary return trip through the area.

Correct placement order for the three scales

Return to the Z Ellis office after obtaining the Pharmacy prescription and Jess’s stethoscope. You should now have every required object:

  • Broken stethoscope
  • Incorrect prescription
  • Zoe’s ID card

Read the underlined lines in the note and place each item in the corresponding scale. The intended left-to-right, first-to-third mapping is:

  1. First scale: broken stethoscope — “I didn’t listen closely enough.”
  2. Second scale: incorrect prescription — “I made mistakes and couldn’t focus on the task at hand.”
  3. Third scale: Zoe’s ID card — “I’m not fit to work in this role any longer.”

After the final placement, a short cutscene confirms completion and advances the story. The next stretch contains more puzzle work, including Jane’s Shed and a task involving blood-pressure, pulse and O2 readings. Why the solution works

The scales note frames each slot as a consequence of professional failure. Rather than requiring obscure symbolic leaps, the game gives you objects that represent each failing through their ordinary function:

  • A stethoscope is used to listen, making it the clearest answer to the first confession.
  • An incorrect prescription is documentary proof of an error, making it the answer to the second.
  • An ID card identifies a worker and their place in an institution, making it the answer to the statement about no longer being fit for the role.

The broader structure is a fetch puzzle: the Clinic establishes the central riddle, map clues unlock two relevant destinations, and each destination provides an item after a smaller access or code challenge. The return to the office then tests whether you connected the objects to the wording rather than simply gathered them.

If you are stuck before the final interaction, check inventory first. Missing the Pharmacy’s locker reward means the poster-and-TV code puzzle is unfinished. Missing the stethoscope usually means the path through St Amelia’s Park, No. 16 and Jess’s back entrance has not yet been completed. If all three items are present, use the clue-to-item mapping above and the scales sequence should resolve immediately.
